Pop-up Form Module with Fade In and Out - Joomla! Forum - community, help and support

i'm having trouble locating module has following features:

• the module must contain html form (for submitting email address newsletter subscription list).
• the module must displayed on selected pages (determined menu item).
• the module must display in fixed position on page such lower right corner or lower left corner. (it still visible if visitor scrolls page.)
• the module must fade in visibility @ time after page loads. (five or ten seconds.)
• the module must fade out after 30 or 40 seconds unless dismissed earlier visitor.
• the module must not fade out if has focus. (don't remove module if visitor in process of filling out form.)
• the module must not interfere operation of page. (it must not block visitor scrolling page or selecting other links on page.)
• the module must not reappear after has been displayed visitor. (the module display disable flag reset after time such 1 hour or 1 day.)

i'm trying present site visitors opportunity subscribe newsletter while minimizing annoyance factor of intrusive pop-up. know of such module or method incorporate joomla website?
