Arrow/Line Note Icon

i updated mountain lion on macbook pro.  i've noticed note icon appears alongside arrows/lines/boxes when add notes them has moved (please see photo).  far away arrow/line/box , interferes other text or pictures on page.  used located directly on arrow/line/box.

 

does know if there fix this? there anyway notify adobe can try update problem?
