Creating form fields

i creating forms using adobe , have 2 questions:

 

1. i've inputted 5 separate text boxes in table , tested out.  seem work except reason, data inputted in first box being automatically copied , repeated in remaining boxes.  how stop happening?

 

2. have inputted 5 date fields go 5 text fields.  i've figured out , located javascript automatically populate today's date.  want form can saved , re-used later similar journal  user open form once, current date populate , make entry in first text field , save.  user open saved form time, date opening populate , make entry in 2nd text field , save.  , on.  possible lock date entry automatically populates when form saved?  if not, there drop down box option date?

1. give fields different names. fields share same name automatically have same value.

 

2. need revise script. can check value of date field, , populate current date if empty. if need this, post current code , tell it's placed.
