Form printing problem

hi,

i have used indesign cs6 create user-fillable form , brought acrobat x pro implement field tab orders, etc.

it's basic form couple of radio button options, pulldown menu, editable text fields , print button.

everything exports correctly idcs6, opens , operates should in acrobat when distribute form user can not input fields nor can print.

i've tested on separate computer , when click print button outputs blank page (no information).

i using mac 10.7.4.
