Newbie question - Assign a link to another page

given pdf page existing text, how assign link page.

 

example:  directory of profiles:

 

name 1

name 2

name 3

name 4

.

.

.

name n

 

i want assign text "name 1" link pdf page contains profile corresponding name.  (i know pdf page number contains profile, can provide link dialog.)
