directory permissions @ 757 unsafe , incorrect. these should 755 , file permissions should 644. report indicates there may file ownership issues. these result of server not using fastcgi. remember ownership , permissions 2 different things.

there number of highlighted apache modules may contributing issues.

it not possible establish mysql information fpa site not installed yet. please confirm version of mysql , collation character set database has been created with.
