it's been while since posted, i'll take stab @ helping nonetheless. it's tough give complete specifics exact printer, here basics printing customized paper size indesign... (i have id 5.5, i'm using give these instructions, concept same cs3 , up).

file–>print.
the "custom" setting grey or unavailable until set new setting...so let's that.
choose printer wish use - in case, epson 1100.
the ppd should fill in automatically, if not, can choose appropriate ppd (the print driver printer).

now need go through windows on left side of screen set rest of print options.

general:

choose number of copies, page range, etc. if have 2 pages in spread , want print them together, check "spreads" box. think rest of page self-explanatory.

setup:

choose paper size here. things can little tricky , you'll have answer few questions yourself... paper size printing on? looks epson 1100 prints on 11x17 , 13x19 sizes. depending on ppd, these sizes may listed in "paper size" drop down menu. if not, can add size using "page setup" button on lower left hand corner of print dialog box. (you'll see warning message pop up, can ignore moment. in new window, under "paper size" choose "manage custom sizes" , enter dimensions of paper. click ok. make sure new size chosen in page setup , click ok again.) in print dialog box....now should have correct size listed under paper size. if original document larger 11x17 or 13x19 indicated, want under "options" section , click on "scale". choose between width (where can choose exact scaling yourself) , "scale fit" (where indesign scaling you). (*i'll note unhelpful if want print @ 12x18 on larger sheet of paper. if that's case, might find faster export document in pdf, resize pages 12x18 , print them out.)

marks & bleeds:

if want include crop marks, registration marks, etc. place it. if want include bleed on print out, can on page well.

output:

looks espon 1100 cmyk printer, window "composite cmyk". if wanted print separations, may have option on page if ppd allows it.

graphics:

images - send data - set "all". may or may not have other options available depending on ppd, set them feel appropriate. postscript level 2 or 3 should fine.

color management:

i've found many people have specific color management preferences of own, set prefer. however, if don't have particular preferences, i'd suggest set color handling "let indesign determine colors". it's you'll have default printer profile show up, , may best go that.

advanced:

opi - grayed out. if it's not, make sure nothing checked.

transparency flattener - if don't have own settings, set high resolution

now, before print, click on "save preset..." , of preferences set available next time want print. "custom" tab @ top have new preset available new settings! ok, print!

i hope helped or helps in future. luck indesign , new mac!
