Complex questionnaire

i have not used forms central before looking solution complex kind of question / answer setup, , save having go through learning curve find out can't done, appreciate tapping knowledge of experienced users. not looking how solve problem, if can done forms central (although offered solutions welcome).

 

i need create questionnaire in each question can have 1 of 5 possible answers. these answers same questions. e.g.

1 always

2 times

3 times

4 rarely

5 never

 

each question can made of one, 2 or 3 parts.

 

the first part of each question can answered 1 of 5 possible answers. if there second part, can answered 1 of 5 possible answers except 1 used answer first part. if there third part, can answered 1 of 5 answers except used answer first , second parts.

 

can forms central handle type of question / answer setup?

formscentral includes likert field type, there no way force form filler select unique choice each question.

After Effect warning: A problem occurred when processing OpenGL commands

i have after effects cs6 in computer cs5.5 installed. 64bit windows 7 ultimate 20gb memory , 2 amd radeon hd 6900 series display adapters drivers updated hour ago (driver date 27.7.2012, version 8.982.0.0).   when trying use after ecffects, error message " after effect warning: problem occurred when processing opengl commands " when clicking composition window.   what done? not much, i'm afraid. need older graphics driver. other disable second card or play configuration. unfortunately not smart person decided cs6 no longer need manual opengl control user, there's no way suppress within program force-revert software-only modes in case of such issues...   mylenium More discussions in After Effects adobe
