how do I make the submit form button appear in the message header?

i'm newbie , i'll try brief, have 2 related questions:

reading documentation, if include submit button on form not place submit form button in message header. first question how discriminate between submit button vs other buttons? believe it's based on action options , if there no url or address send form in action settings treats "non-submit" button, , place submit button in message header. correct?

 

secondly, i'm getting no submit button in message header regardless inclusion of button on form itself. there else must done form upon saving/other drives inclusion of message header submit button? posting form on staff web page (link file), open it, fill out, "submit" it. i've tried variations of save "reader extended" versions no luck well. help!

the button in purple bar @ top appear when put form through distiribute form process. if placed submit button on form somewhere, it's action modified matches button in purple bar does. it's possible add same javascript document distribute form process cause button appear, unusual.
