Sudden, and regular crash issue using brushes/Wacom Cintiq

i'm using photoshop cs5 extended 64bit (12.0.4) on win7 professional (64 bit) , have had issue using cintiq 12wx tablet in using brush tablet (whether i'm using tablet monitor, or laptop monitor) causes photoshop crash, within first 30 seconds of usage.  has become regular expect crash , no longer can trust use, @ same time depend on use work done, expedient solution absolutely necessary.  not crash if use brush mouse, that's not solution either, can imagine.  have been using tablet photoshop long time without incident, , can think of no changes system i've made in period when began happening.

can recognize these symptoms , recommend course of action?

 

any , possible expedient issue appreciated.  in advance...

have checked see if there update wacom tablet?
